The Case Manager will provide person-centered support to individuals with intellectual and developmental disabilities in their homes and communities. Responsibilities include monitoring services, leading team meetings to develop Person Directed Plans, and responding to crisis situations.
Candidates must hold a Bachelor's degree from an accredited college or university. A valid Texas driver's license and insurability under the agency vehicle policy are also required.
