Provide case management, crisis intervention, and care coordination for justice-involved individuals with co-occurring psychiatric and substance use disorders. Coordinate release plans from jail settings and facilitate connections to community supports and diversion courts.
Requires a Bachelor's degree in psychology, sociology, social work, or a related human services field. Candidates must have 1-2 years of behavioral health case management experience and a valid Texas driver's license.
