The RN Case Manager provides direct skilled nursing care and case management to patients within their homes. Responsibilities include developing treatment plans, performing wound care, and coordinating with a clinical team to achieve positive patient outcomes.
Candidates must have at least an Associate degree in Nursing, a current California RN license, and CPR/BLS certification. A minimum of 3 years of RN experience in an institutional setting such as acute care, rehab, or home health is required.

Guaranteed base salary plus competitive per visit rates and performance incentives! (For Full-time employees only)
Location: Torrance, CA and surrounding areas
Sign-On bonus: $5,000 (For Full-time employees only)
The Neogen Care Home Health Registered Nurse provides direct patient care in the patient's home under the supervision of a physician. Responsibilities include following start of care plans, providing treatment, discharges, wound care, medication administration, and working collaboratively with a clinical team to achieve positive patient care outcomes.
