The Wellness Coordinator manages a caseload to assist clients with medical and mental health service coordination and wellness goal identification. They are responsible for completing comprehensive assessments, developing service plans, and maintaining accurate clinical documentation in an electronic health record system.
Candidates must hold a Bachelor's degree in social work, psychology, social services, or a related field. Alternatively, a Bachelor's degree in an unrelated field with one year of equivalent social service experience is accepted.
Description Health Affiliates Maine is accepting resumes for per diem Behavioral Health Home (BHH) Care Coordinators to support children and families within York county. The BHH program is an integrated, collaborative, community-based care model for the whole person. Services provide assistance to children/families with mental health, medical and other resource needs.
As the Care Coordinator, you will work alongside a Peer Support, Nurse Care Manager and BHH members to assist clients manage and coordinate medical and mental health services and also identify the resources needed to support wellness goals.
Maintain an understanding of the core requirements of the Behavioral Health Home. Provide care that integrates both physical and mental health. Complete comprehensive assessments of service care needs, develop service plans and also maintain accurate client files. Manage a caseload independently and effectively. Collaborate with external community-based services and providers. Assist in negotiating referrals and linkages to providers and other services as needed. Enter clinical documentation into an electronic health record system. Perform other duties as assigned. Benefits (Include but are not limited to the following):
