The Community Health Worker II manages a caseload of clients to support care coordination, intake processes, and treatment adherence. They conduct outreach to address gaps in care, reduce no-show rates, and facilitate access to necessary health and social services.
A high school diploma or equivalent is required, with an associate or bachelor's degree preferred. Candidates should have experience in behavioral health, social services, medical assisting, or healthcare.
LICENSURE & EDUCATION: Required: High school diploma or equivalent Preferred: associate or bachelor’s degree in health, social services, or related field preferred.
Minimum of 3 years full-time experience as a Care Navigator, Community Health Worker, or comparable care coordination role. Must obtain Kansas CHW certification within 6 months of employment if not already certified (required).
Preferred: Current Missouri or Kansas CHW certification
Experience in behavioral health, social services, or healthcare settings is strongly preferred.
