The Patient Navigator will help patients overcome barriers to care and connect them with essential community resources. They will work within an interdisciplinary team to improve health outcomes and reduce avoidable hospitalizations.
Candidates must have a High School Diploma or GED and at least 2 years of experience in health or social services. A valid Hawaiʻi driver’s license, insurance, and reliable transportation are also required.
