The Physical Therapist will provide skilled therapy services in patient homes, including evaluations, individualized treatment planning, and functional rehabilitation. They will collaborate with interdisciplinary teams to ensure compliance with Medicare guidelines and maintain accurate clinical documentation.
Candidates must hold a Bachelor's degree or higher from an accredited Physical Therapy program and possess a current Connecticut Physical Therapist license. A minimum of 2 years of clinical experience in home health or outpatient settings is required.

We are hiring Physical Therapists for Full-time, Part-time or Per-Diem work schedules.
Provide skilled physical therapy services to patients in their homes under a home health plan of care. This role focuses on short-term, intermittent care for patients recovering from illness, injury, or hospitalization.
In-Home outpatient physical therapist (Ihop – medicare part b)
Deliver outpatient-level physical therapy services in patients' homes and assisted living communities. This role often includes ongoing therapy for post-surgical patients and those requiring longer-term rehabilitation.

The full time pay range for this position is $90,000 to $110,000 per year and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
