The Hospice RN provides direct patient care and comprehensive case management for individuals in homes or care facilities. This includes conducting assessments, implementing individualized care plans, and coordinating with an interdisciplinary team to ensure patient dignity and comfort.
Candidates must be a graduate of an accredited nursing program with a current Connecticut or Compact RN license and at least 2 years of nursing experience. A BSN and prior experience in hospice or home health are strongly preferred.
Graduate of an accredited nursing program; BSN preferred Current RN license in the State of Connecticut or Compact license. 2+ years of nursing experience; hospice or home health experience strongly preferred Ability to work independently and respond effectively in crisis situations Strong clinical judgment and interpersonal communication skills Comfortable with electronic health records and mobile documentation Valid driver's license, reliable transportation, and willingness to travel within service area/territory Current CPR certification Must complete RVNAhealth's hospice training program upon hire
