The Case Manager serves as the primary advocate for justice-involved members, facilitating medical referrals, setting goals, and managing crises. They conduct community outreach and maintain active engagement to ensure proper coordination of care for members with chronic conditions.
Candidates must have at least one year of experience in a behavioral healthcare setting and experience working with individuals involved in the justice system. A valid driver's license and insurance are required, along with the ability to work a flexible schedule including evenings and weekends.

Seeking a full-time Case Manager to join our multidisciplinary team for CalAIM Enhanced Care Management. This program offers in-person case management and support services to Medi-Cal members with multiple chronic conditions.
include but are not limited to serving as the main point of contact and advocate for Enhanced Care Management for the Justice Involved (JI) enrolled members. This position involves connecting enrolled members to community resources, facilitating medical referrals, setting goals, addressing barriers, linking to health and support services, establishing benefits, providing transportation, managing crises, and conducting outreach within the community and TTC sites. The position also requires maintaining active engagement and interactions with program-enrolled members to ensure proper coordination of care.
A minimum of 1 year experience in case management is required, along with a background in working with individuals who have mental health conditions and substance use disorders. Candidates must have a flexible schedule that includes late evenings and weekends. Candidates must have experience with in-home visits, homeless encampments, and correctional facilities. The ideal candidate should have either personal experience with the justice system or a proven history of working with individuals who are involved with the justice system.
HOURS: 8 per Day / 5 Days per Week
• 1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff
3. Bilingual English/Spanish preferred
4. Valid driver’s license and insurance
For AOD programs, registration or certification with a DHCS approved state level counselor certification agencies.
