The HIV/AIDS Case Manager facilitates individualized case management services including intake, assessment, care planning, and psychosocial referrals. They also provide health education, HIV testing, and advocacy for HIV-positive clients within the community.
Candidates must be bilingual in English and Spanish and hold a bachelor's degree in Social Work, Public Health, or Social Sciences, or have three years of related experience. At least one year of recent experience in a healthcare setting is required, along with proficiency in Microsoft Office and electronic health record systems.

Your Role & Impact
The HIV/AIDS Case Manager facilitates a collaborative process to provide individualized case management services to HIV-positive clients that includes intake, assessment of client needs, care planning and psychosocial referrals, advocacy, partner services, HIV testing, and health education.

The pay range for this role is $25.30 - $35.42 on an hourly basis based on experience and internal equity.
