Position Options Home Health Physical Therapist (Medicare Part A) Provide skilled physical therapy services to patients in their homes under a home health plan of care.
This role focuses on short-term, intermittent care for patients recovering from illness, injury, or hospitalization.
Key Responsibilities: Perform start-of-care visits, evaluations, reassessments, and discharges Develop and implement individualized treatment plans Provide functional rehabilitation to improve mobility and independence Collaborate with nurses, physicians, and interdisciplinary care teams Complete OASIS documentation and maintain compliance with Medicare Part A guidelines Educate patients and caregivers on safety, fall prevention, and disease management In-Home Outpatient Physical Therapist (IHOP – Medicare Part B) Deliver outpatient-level physical therapy services in patients' homes and assisted living communities.
This role often includes ongoing therapy for post-surgical patients and those requiring longer-term rehabilitation.
Key Responsibilities: Conduct comprehensive outpatient evaluations and develop care plans Treat patients recovering from orthopedic procedures (e.g., total joint replacements) Provide ongoing therapy focused on strength, balance, and functional mobility Perform pre-operative assessments and post-surgical follow-up care Collaborate with physicians and care teams to optimize outcomes Maintain Part B documentation and billing compliance Qualifications (Both Roles) Bachelor's degree or higher from an accredited Physical Therapy program Current Connecticut Physical Therapist license Minimum of 2+ years of clinical experience (home health and/or outpatient preferred) Strong clinical, communication, and organizational skills Experience working in home settings or assisted living communities is a plus Why RVNAhealth?
