The Occupational Therapist evaluates and treats residents, develops individualized treatment plans, and maintains accurate medical documentation. They also supervise therapy assistants and collaborate with the interdisciplinary team to ensure optimal resident outcomes.
Candidates must hold a Bachelor's degree in Occupational Therapy and a current, active California Occupational Therapist license. Experience in skilled nursing or geriatric settings is preferred but not required.
Evaluate residents within 24 hours (or one business day) of physician referral; document delays appropriately Develop individualized treatment plans and obtain physician approval for services Provide occupational therapy treatment per the physician-approved plan of care Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in accordance with the state practice act Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams Participate in patient care conferences, utilization review meetings, and rehab meetings as needed Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements Educate residents, families, and caregivers on safety, adaptive techniques, and home programs Participate in discharge planning and coordination of care Recommend and facilitate ordering of durable medical equipment (DME) to promote functional independence Conduct in-services and training for facility staff as assigned Report equipment issues promptly to ensure safe and effective operation Maintain professional, respectful relationships with residents, families, and staff Supervisory Responsibilities Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students Qualifications Education & Experience Bachelor’s degree in Occupational Therapy required Master’s or Doctorate degree in Occupational Therapy preferred Skilled nursing or geriatric experience preferred but not required Licensure & Certifications Current, active California Occupational Therapist license in good standing Must meet all continuing education requirements per state regulations Skills Proficient in electronic documentation systems (Casamba and PointClickCare preferred) Strong clinical judgment, communication, and organizational skills Ability to work independently and as part of a collaborative team Physical Requirements Frequent standing, walking, pushing, pulling, and reaching Ability to lift and move up to 50 pounds frequently Occasional stooping, kneeling, crouching, and balancing Visual and auditory acuity sufficient for patient care and documentation Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions Work Environment Skilled nursing and short-term rehabilitation setting Noise level typically low to moderate One primary work location Equal Employment Opportunity (EEO) Statement Visalia Post Acute is an Equal Opportunity Employer.
