The COTA will provide direct patient-centered occupational therapy services and implement treatment plans under the supervision of an Occupational Therapist. They are also responsible for documenting patient progress and collaborating with the interdisciplinary care team to ensure effective discharge planning.
Candidates must hold a current, active California Certified Occupational Therapy Assistant (COTA) license and have graduated from an accredited program. Experience in skilled nursing or geriatric settings is preferred, along with proficiency in electronic documentation systems.
Provide treatment to residents as directed by the supervising Occupational Therapist Follow physician-approved treatment plans and modify activities as appropriate within scope of practice Record daily treatment notes, weekly progress notes, and recertification documentation per OT Board, facility, and payer requirements Communicate resident progress, challenges, and care plans with the Occupational Therapist and interdisciplinary team Educate residents, families, caregivers, and nursing staff on home programs, safety techniques, and discharge planning Participate in discharge planning and coordination of care Assist nursing department with training of Restorative Aides as needed Participate in in-service education and facility training programs Maintain clean, safe, and organized treatment areas Report equipment issues promptly to ensure safe and effective operation Maintain professional, respectful relationships with residents, families, and staff Supervisory Responsibilities This position has no supervisory responsibilities Qualifications Education & Experience Graduate of an accredited Occupational Therapy Assistant program Skilled nursing or geriatric experience preferred but not required Licensure & Certifications Current, active California Certified Occupational Therapy Assistant (COTA) license in good standing Must meet all continuing education requirements per state regulations Skills Proficient in electronic documentation systems (Casamba and PointClickCare preferred) Strong communication, organizational, and clinical skills Ability to work independently and collaboratively within a team environment Physical Requirements Frequent standing, walking, pushing, and pulling Ability to lift up to 50 pounds frequently and exert greater force as required for patient care Occasional kneeling, crouching, stooping, and balancing Visual and auditory acuity sufficient for patient monitoring and documentation Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions Work Environment Skilled nursing and short-term rehabilitation setting Noise level typically low to moderate One primary work location Equal Employment Opportunity (EEO) Statement Visalia Post Acute is an Equal Opportunity Employer.
