Service Coordinators provide flexible, community-based support to promote stability and recovery for individuals facing mental health and substance use challenges. They manage a caseload, assist with housing and life skills, and maintain detailed documentation of client interactions.
Candidates must have a valid driver's license, a reliable vehicle, and the ability to lift 50 pounds. A strong understanding of mental health, substance use issues, and a client-centered, harm-reduction approach is required.

Service Coordinators are members of the interdisciplinary team who provide flexible, compassionate, community-based services that are designed to promote the stability, recovery, and community reintegration of individuals who experience mental health challenges, histories
of homelessness, and may also have challenges around substance use. Service Coordinators function as integral members of Pathways’ Assertive Community Treatment (ACT) team which utilizes an interdisciplinary staff to serve a combined caseload of clients and communicates with community partners.
