Job Description

Summary The Registered Nurse Case Manager is responsible for coordinating and delivering hospice care to patients and families in their homes and community settings. This role includes conducting hospice admissions, comprehensive assessment, care planning, case management, patient and family education, and collaboration with the interdisciplinary hospice team to ensure high-quality, compassionate, and compliant end-of-life care.

• Essential Job Functions/Responsibilities Conduct comprehensive assessments of patients and families to identify hospice needs, including physical assessments, symptom evaluation, and review of medical history.
• Perform hospice admissions and start-of-care visits, including comprehensive assessments, verification of hospice eligibility, patient and family education, and completion of all required admission documentation in compliance with regulatory standards.
• Develop, implement, and regularly update individualized plans of care based on nursing assessments, patient goals, and physician orders.
• Serve as the primary Case Manager for assigned patients, coordinating services and ensuring continuity of care across the interdisciplinary team.
• Administer medications and treatments as prescribed and evaluate patient response to interventions.
• Provide ongoing pain and symptom management and adjust interventions in collaboration with the physician and hospice team.
• Provide education to patients and families regarding disease progression, medications, symptom management, safety, and care techniques.
• Maintain accurate, timely, and thorough clinical documentation that reflects patient status, care provided, and outcomes.
• Communicate changes in patient condition promptly to physicians and interdisciplinary team members.
• Promote and maintain a safe care environment for patients and families.
• Provide emotional support and counseling to patients and caregivers throughout the hospice journey.
• Collaborate with social workers, chaplains, hospice aides, and other interdisciplinary team members to address patient and family needs.
