The RN Case Manager leads and coordinates care throughout the hospice journey while building meaningful connections with patients and families. They collaborate with an interdisciplinary team to address the physical, emotional, and spiritual needs of patients in the field.
Candidates must be graduates of an accredited nursing school with a current Colorado RN license and CPR certification. A minimum of 1-2 years of experience in hospice, palliative care, home health, or related clinical fields is required.

Salary: $85,508 – $93,620 annually
