The Care Coordinator assists families in identifying strengths and needs while developing and monitoring individualized care plans for youth. They also facilitate team meetings, provide advocacy, and coordinate services to ensure access to necessary resources.
Candidates must hold a Bachelor's degree in a human services related field and have at least 2 years of experience in a mental health setting. A valid driver's license is required, and CPR/First-Aid certification must be obtained within 6 months of hire.

About the program: Care Coordination
Care Coordination at CHR is an intensive program for children and youth ages 0-18 (or up to age 22 if in school) with emotional/behavioral difficulties and complex service needs and their families. Care Coordinators assist the family through a child-specific team that develops individual and family-focused service plans. The coordinators also provide ongoing consultation with the child-specific team to provide linkage, coordination, follow-up, and advocacy for the child and family
Bachelor’s degree in human services related field required.
Minimum of 2 years' experience in mental health setting.
Valid driver’s license. CPR/First-Aid Certification within 6 months of hire.

$20.30/hr min, and up commensurate upon experience.
*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
