The Physical Therapist is responsible for providing comprehensive physical therapy services to patients with chronic pain and functional impairments. This includes developing individualized treatment plans and collaborating with an interdisciplinary healthcare team.
Candidates must be graduates of an accredited Physical Therapy program and hold an active license in Illinois. A minimum of 2 years of clinical experience is preferred, along with proficiency in EMR documentation and strong communication skills.

We are seeking a licensed Physical Therapist (PT) to join our interdisciplinary pain management team. This role is responsible for providing comprehensive physical therapy services to patients with chronic pain and functional impairments in an outpatient clinical setting.
The Physical Therapist collaborates closely with providers, medical assistants, and behavioral health staff to design and implement individualized treatment plans focused on improving mobility, reducing pain, and restoring function.
Candidates with experience in pain management, orthopedic rehabilitation, functional medicine, or integrated medical practices are strongly encouraged to apply.
$40.00 - $60.00 per hour
