Candidates must have at least 2 years of acute care hospital case management experience and an active California RN license. Proficiency in Epic EMR, care coordination, and discharge planning is strictly required.

This role supports an acute care hospital case management team focused on care coordination, utilization review, and discharge planning across a diverse patient population.
POSITION OVERVIEW This assignment is ideal for experienced inpatient Case Managers who are confident managing high caseloads, coordinating care across interdisciplinary teams, and ensuring appropriate utilization of healthcare resources.
