The Transitional Care Management Nurse acts as an intermediary between medical teams and patients to ensure effective discharge planning and follow-up. They are responsible for monitoring appointment adherence, managing TCM phone processes, and applying appropriate E&M codes to improve patient outcomes and reduce readmissions.
Candidates must be graduates of an accredited School of Nursing and hold a valid, unencumbered Mississippi nursing license. A minimum of two years of direct patient care or clinic experience is required, with preference given to those with case management experience.

Job Description: Summary
Position Overview: A Transitional Care Management Nurse will function as an intermediary between the medical teams and the patients. In this capacity, the role is designed to accomplish the following items for all three campus locations within SRHS: first line support of appointment dates and times given to the patients before discharge with a goal of 95%, completion of part 1 of the TCM phone call process with a goal of 90%, and provide applicable E&M codes to providers based on insurance coverage. Acute awareness is needed to prevent unnecessary readmissions, improve HCAHPS, drive positive discharge outcomes and reduce LOS, and review ER utilization using proactive approaches to any barriers.
