The Case Manager provides behavioral health services, including assessments, recovery planning, and skills training for adults with mental health diagnoses. They collaborate with a multi-disciplinary team to deliver care in home and community settings while maintaining professional documentation.
Candidates must hold a Bachelor's degree in a human services field such as psychology, sociology, or social work. A valid Texas driver's license and the ability to be insurable under agency policy are also required.
