The Case Manager identifies and coordinates resources to meet the individualized needs of program participants while implementing service and treatment plans. They also provide community-based support, facilitate access to services, and maintain accurate documentation in compliance with organizational standards.
Candidates must have a minimum of two years of experience in behavioral health, addiction recovery, or IDD services. A bachelor's degree in Human Services or a related field is preferred, along with a valid driver's license and reliable transportation.

The Case Manager is responsible for identifying and coordinating resources to meet the individualized needs of program participants. This role ensures the effective implementation of each participant’s service and treatment plan, serves as an advocate to facilitate access to needed services, and identifies program needs and service gaps while recommending strategies for improvement. The Case Manager must demonstrate a strong commitment to APIS and RHD values in all aspects of their work.
