The Speech Language Pathologist will provide skilled therapy services, conduct comprehensive evaluations, and develop individualized treatment plans for patients in their homes. They will also collaborate with interdisciplinary teams and maintain accurate, timely clinical documentation.
Candidates must hold a Master’s or Doctoral degree in Speech Language Pathology and possess a current Texas SLP license. Strong organizational skills and the ability to travel throughout the San Antonio area are required.
Touchstone Home Health is seeking a Speech Language Pathologist (SLP) to join our San Antonio team—where clinical excellence meets purpose-driven care. Key Responsibilities Provide speech therapy services in accordance with physician orders and individualized care plans Conduct comprehensive evaluations of communication and swallowing needs Develop and adjust treatment plans to support patient progress and safety Educate patients, families, and caregivers on home-based strategies and exercises Complete timely and accurate documentation, including OASIS assessments and care updates Collaborate with physicians and interdisciplinary teams to coordinate high-quality care
