Manage the acute, subacute, and primary care needs of patients by rounding at assigned facilities. Collaborate with the OnSite team, nursing home staff, and families while providing patient education and integrating non-pharmacological interventions.
Requires a Master of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification. Must hold an active South Carolina license and DEA registration, with at least one year of experience preferred.
Monday - Friday visiting schedule with flexibility
