The Case Manager is responsible for conducting comprehensive assessments, developing individualized care plans, and facilitating access to necessary medical and community services. They also serve as a liaison between patients, families, and the clinical team to ensure a cohesive and supportive treatment environment.
Candidates should possess a bachelor's degree in a human services field and have experience in behavioral health or substance use treatment. Essential requirements include CPR certification, CPI training, and the ability to pass a Level II background check.

The Case Manager ensures a safe and supportive environment for patients throughout treatment. This role assists with orientation, admissions, discharge planning, and documentation, while serving as a liaison between patients, the clinical team, and admissions staff.
