The RN Clinic delivers direct patient care, care coordination, and health education within an ambulatory community health clinic. Responsibilities include performing clinical procedures, managing acute and chronic conditions, and supporting population health initiatives through evidence-based practice.
Candidates must hold an Associate degree and a current, unrestricted Oregon RN license, along with BLS/CPR certification. A minimum of two years of clinical experience in acute or ambulatory care is preferred.
The RN Clinic role is a licensed registered nurse who delivers direct patient care, care coordination, and health education within an ambulatory community health clinic setting. This role collaborates with providers and the interdisciplinary team to assess, plan, implement, and evaluate care for patients across the lifespan. The RN Clinic role is responsible for performing clinical procedures, managing acute and chronic conditions, and supporting population health initiatives. The position emphasizes patient advocacy, evidence-based practice, and a commitment to quality and safety in the outpatient environment.
Minimum two years of RN clinical experience in acute or ambulatory care preferred. Care coordination experience in an ambulatory setting preferred. Experience in nurse-led patient visits preferred (i.e. diabetes and chronic disease education, anticoagulation management)
Pediatric RNCC: Experience working with pediatric patients preferred
