The Clinical Pharmacist coordinates medication therapy management and collaborates with providers to ensure safe, effective, and cost-efficient pharmacotherapy. They also provide drug information, educate clinical teams, and maintain accurate electronic health records for patients.
Candidates must possess excellent communication skills, be detail-oriented, and have the ability to work independently while collaborating with a team. Proficiency in electronic medical records systems and MS Office is required to manage patient care and documentation.
Join Mosaic's integrated care team as a Clinical Pharmacist — a pivotal role at the intersection of patient care, provider collaboration, and medication safety. Working across all Mosaic clinics, you'll be embedded in multidisciplinary teams alongside providers and nursing staff to ensure every patient receives medication therapy that is safe, effective, and cost-conscious.
In this role, you will assess, implement, and monitor medication-related therapies — catching therapeutic incompatibilities, supporting adherence, and engaging patients through face-to-face, telephonic, and electronic care models. You'll serve as a trusted drug information resource for care teams, deliver education to patients and staff, and help shape the future of patient-centered pharmacy practice within our growing system.
The Clinical Pharmacist will provide care to the patients of Mosaic Community Health 4 days per week (10 hour days). 2 days a week will be in our Conners and Madras clinics and 2 days a week will be Telehealth visits.
Skills & Knowledge
Must have excellent oral and written communication skills, be customer service oriented, and have ability to interact with a diverse patient population in a professional manner. Must be detail-oriented, have ability to multi-task and work independently in making pharmacy and drug-related judgments, while still collaborating with team members. Must be organized and use analytical skills in problem solving and decision making. Basic to intermediate experience in computer applications, specifically electronic medical records system and MS Office a must.
