The provider will build a patient panel and deliver comprehensive, patient-centered care across the lifespan. Responsibilities include diagnosing and treating acute conditions, managing chronic illnesses, and providing preventive services such as screenings and immunizations.
Candidates must be graduates of an accredited Nurse Practitioner or Physician Assistant program and be eligible for Minnesota licensure. Prior experience in Family Medicine, Walk-In Clinics, Emergency Medicine, or Urgent Care is required.

This full-time role offers the opportunity to practice independently in a collaborative primary care setting, diagnosing and treating acute conditions, managing chronic illnesses, and providing preventive care such as routine exams, screenings, immunizations, and lifestyle counseling. $109,241.60 - $124,904 per year Pay may be determined by factors such as education, experience, skills, knowledge, location, and internal equity Salary and salary range are based on a 1.0 FTE, reduced FTE will result in a prorated offer rate May be eligible for additional pay through bonuses, quality incentives, or production-based (RVU) compensation.
