The Case Manager is responsible for assessing, planning, and coordinating patient care to meet health needs across the continuum of care. They collaborate with the healthcare team to ensure appropriate service utilization and optimal patient outcomes.
Candidates must hold a 2-year Nursing degree and a valid California Registered Nurse license. A minimum of two years of nursing experience is required, with pediatric and case management experience preferred.
