The Intake/Discharge Coordinator manages the end-to-end clinical intake and discharge processes, including assessments, documentation, and client orientation. They collaborate closely with clinical and admissions teams to ensure a supportive experience and maintain continuity of care.
The ideal candidate has at least two years of experience in behavioral healthcare or residential treatment and a bachelor's degree. Strong organizational, communication, and interpersonal skills are required to manage multiple priorities in a fast-paced environment.
Coordinate with the Clinical Director, Admissions team, and clinical staff to manage the intake process from start to finish Complete intake assessments and ensure all admissions documentation is accurate and complete Welcome new clients and orient them to the treatment milieu Conduct facility tours, phone assessments, and additional admissions-related tasks as needed Communicate professionally and compassionately with clients, families, referral sources, and internal team members Support outcomes research initiatives by gathering client feedback, reviews, and assessment data Coordinate and ensure completion of all discharge procedures, including returning client belongings and finalizing documentation Collaborate with clinical and admissions teams to ensure timely, organized, and supportive discharges Support continuity of care through clear communication and documentation Maintain accurate and timely clinical documentation and support chart audits as needed Participate in treatment team meetings, staff meetings, and in-service trainings Assist with onboarding and training for staff involved in intake and discharge processes Demonstrate professionalism in appearance, communication, and conduct in alignment with agency values and philosophy
Bachelor’s degree preferred; equivalent education and relevant experience considered Minimum of 2 years of experience in residential treatment, facilities management, or behavioral healthcare preferred Minimum of 1 year of experience working with youth and families Strong organizational skills with exceptional attention to detail Ability to manage multiple priorities in a fast-paced environment Excellent verbal and written communication skills, particularly in emotionally sensitive situations Proven ability to collaborate effectively with multidisciplinary clinical teams
