The LPN/RN will coordinate anticoagulation therapy for clinic patients, including monitoring lab results, adjusting dosages, and providing patient education. They will also document clinical data, manage prescription refills, and communicate effectively with providers and home health groups.
Candidates must be a graduate of an accredited nursing school and hold a current Massachusetts RN or LPN license. Basic Life Support certification and strong interpersonal and computer skills are required for this clinic-based role.
