What we’re looking for • Education

• High School diploma; Medical Assistant or higher preferred
• Experience: Two years of experience in the ambulatory healthcare setting; Ideal candidate will have prior experience in population health initiatives such as chronic disease management, care management, or utilization management
• Skills and
• Abilities: Ability to work independently, setting priorities to coordinate care plan efficiently; Ability to work effectively in a team environment; Efficient with MS Office – Outlook, Word, Excel, Teams
