The Nurse Practitioner will provide hospice and palliative care to seriously ill patients to alleviate symptoms and align treatments with patient values. They will collaborate with a clinical team to deliver care in patient homes and serve as an active member of the medical community.
Candidates must have at least 2 years of experience in chronic disease management or long-term care within the last 5 years. A nursing degree from an accredited school is preferred, along with the ability to perform physical tasks like lifting 80-100 lbs.
A heart to serve patients and families and a passion for providing the best possible care Education: Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing preferred Experience: 2 years experience within the last 5 years in chronic disease management/long-term care. Hospice background preferred Required: Reliable transportation.
