The Case Manager assesses client needs, develops individual service plans, and links clients to necessary housing and supportive services. They are responsible for conducting intakes, providing crisis intervention, and documenting all case management activities according to agency guidelines.
A Bachelor's degree in Social Work or a Human Services field is preferred, along with at least one year of experience working with vulnerable populations. Candidates must possess strong organizational skills, proficiency in Microsoft Office, and the ability to work a flexible schedule including evenings and weekends.
