The Case Manager II coordinates and facilitates patient care across the continuum of care by collaborating with the Interdisciplinary Care Transitions team, ensuring optimal resource utilization and compliance with review agencies. This role involves comprehensive assessment, care planning, implementation, and evaluation to enhance patient management quality and cost-effectiveness.
Candidates must have experience in healthcare case management, utilization review, or discharge planning, and possess an educational background in nursing or social work (BSN, MSN, BSW, or MSW from an accredited school). Essential knowledge includes payor practices, Medicare benefits, and accreditation standards, alongside strong critical thinking and interpersonal skills.
