The RN Case Manager provides individualized end-of-life care and case management for terminally ill patients in their homes or community settings. They collaborate with an interdisciplinary team to develop care plans, facilitate admissions, and provide clinical support.
Candidates must be a graduate of an accredited nursing school and hold a current Minnesota RN license. A minimum of three years of recent nursing experience in acute, long-term, or home care/hospice is required.

The RN Case Manager (RNCM) is someone whose dedicated clinical expertise results in terminally ill patients receiving exceptional end-of-life care. In this role, the RNCM collaboratively works alongside an interdisciplinary team to provide end-of-life care to those needing hospice services in their home or community setting. The RNCM provides individualized case management, admission facilitation, clinical support, continuous care, after hours call, and/or other skilled nursing expertise with chronic and end-of-life care.
Graduate of accredited school of nursing Current state of Minnesota RN licensure Three years recent nursing experience in acute, long-term care or home care/hospice/public health Patient care planning and patient teaching skills Written and verbal communication skills Critical thinker Current Minnesota driver’s license, insurance and daily access to reliable vehicle Computer literacy
