The Mental Health Rehabilitation Specialist provides direct community support, case management, and crisis intervention services using recovery-based practices. They collaborate with various agencies and healthcare services to coordinate care and provide education to members and their families.
Candidates must hold a degree in a social or human services field ranging from an Associate's to a Master's degree. Relevant experience in mental health services is required, ranging from two to six years depending on the level of education.

The Mental Health Rehabilitation Specialist provides direct members served community support and case management services to assigned members served using Psychiatric Rehabilitation principles, Recovery Philosophy, and Evidence-Based practices. This position provides education and support to members served and their families. S/he coordinates service needs and collaborates with other services and agencies such as the Conservator's Office, Public Defender's Office, Child and adult Protective Services, Probation Department, and Inpatient Services, Long Term Care Services, Residential Settings and County Mental Health Services. The Mental Health Rehabilitation Specialist completes assessments, provides mental health rehabilitation services, case management, crisis intervention, integrated whole person care, and trauma informed care.
Shifts Available: On-Call | Shifts and days vary as needed
Expected starting wage range is $27.85 - $34.42. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

Alameda County Youth CSU is a Crisis Stabilization Unit serving youth populations within Alameda County. These programs provide screening/triage, medication support and brief treatment in a safe, protective setting. These programs are sometimes co-located with acute inpatient programs, allowing for a 'warm hand-off' transfer when needed. The typical length of stay is up to 24 hours.
