The Physical Therapist Assistant provides skilled physical therapy services in patient homes under the direction of a Physical Therapist. Responsibilities include performing prescribed therapy procedures, documenting patient progress, and educating families on safety and assistive devices.
Candidates must hold a current Physical Therapist Assistant license and have at least one year of experience in an acute care setting. A valid driver's license and auto insurance are required for travel to patient homes.
We provide the tools and support needed for success, along with competitive financial incentives: Guaranteed base hourly rate Generous uncapped bonus structure Mileage reimbursement Additional bonus opportunities Flexible scheduling Full team support (scheduling, patient admissions, etc.) Benefits Medical, dental, and vision insurance (customizable) Paid time off (PTO) 401(k) participation Company-paid life insurance Employee Assistance Program (EAP) Qualifications & Attributes Current Physical Therapist Assistant license in the state of employment Minimum 1 year of PTA experience in an acute care setting (required) Home care experience (preferred) Strong communication skills with patients, families, physicians, and coworkers Knowledge of therapy best practices Ability to make sound clinical judgments Problem-solving skills with a solution-oriented mindset Detail-oriented and observant Strong organizational and time-management skills Valid driver’s license and auto insurance (listed driver) Major Areas of Responsibility Patient Care Provide skilled services in patients’ homes per Physical Therapist plan of care Perform prescribed therapy procedures and diagnostic tests Treat patients to relieve pain and restore function using physical methods (exercise, massage, heat, water, etc.) Assess functional limitations and support progress toward goals Recommend or adjust prosthetic/orthotic devices Monitoring & Communication Observe, record, and report patient reactions, changes, and progress Communicate effectively with physicians, therapists, and care team Participate in case conferences to coordinate care Education & Support Educate patients and families on therapy techniques and safety Instruct on use of assistive devices (wheelchairs, walkers, braces, etc.) Support patient independence and reduce reliance on care services Documentation & Compliance Complete all clinical documentation per agency and Medicare/Federal guidelines Document treatments, assessments, and patient education Participate in chart audits as requested Operations Communicate scheduling changes promptly Attend trainings, meetings, and required in-services Assist with onboarding and training of new staff Essential Functions Travel to patient locations as scheduled Maintain consistent attendance at assigned visits Complete documentation within required timeframes: 24 hours for routine visits 48 hours for start-of-care visits Follow plan of care within PTA scope of practice Manage all aspects of visit-related responsibilities, including: Preparation Travel Documentation Communication with care team Health Qualifications Must meet all physical job requirements Provide proof of negative TB test (or complete testing at orientation) Annual TB testing required thereafter Must be free of conditions limiting ability to perform essential duties May be exposed to bloodborne pathogens Physical Demands Constant (66%–100%) Reading, writing, and speaking English Communication and computer use Manual dexterity Hearing and vision (including close work) Walking, sitting, standing, bending In-person and phone communication Frequent (34%–66%) Driving Lifting up to 50 lbs (with or without assistance) Stretching, reaching, climbing Stooping Occasional (2%–33%) N/A Rare (1% or less) N/A #INDSWG1 We’ll help you put your passion for patient care to work.
