The Case Manager provides oversight and care management, including assessments, treatment planning, and rehabilitation services for clients. They work within the community, clinic, and home settings to support individuals at risk for early psychosis.
Applicants must have a high school diploma with six years of experience, an associate degree with four years of experience, or a bachelor's degree with two years of experience in the mental health field. A valid California driver's license and reliable transportation are required for community-based service delivery.
