The RN Case Manager will assess patient needs, create personalized hospice care plans, and provide direct nursing care including medication administration and symptom management. They will also collaborate with an interdisciplinary team and maintain accurate documentation while supporting patients and their families.
Candidates must be a graduate of an accredited nursing school with a current nursing license and at least one year of nursing experience. A valid driver's license and an insured, reliable vehicle are also required for travel within the assigned territory.
