Heart Failure Nurse Navigator-Cardiovascular Admin- 40HR Day Shift (M-F, 8:30a-5p)

• In conjunction with the Heart Failure team assumes responsibility for assessment, coordination, and education of the Heart Failure patient, on the inpatient and outpatient settings.
• Assists with delivering high quality, efficient medical care to patients in the Heart Failure Clinical Excellence Program within the Temple Heart and Vascular Institute.
• Works with complex heart failure patients and supports both the physicians and advance practitioners with patient care management.
• This includes assessments, interventions, referrals, documentation, telephone triage and pre-cert registration processes.
• Provides ongoing health education and counseling activities; prompt abnormal testing reporting and follow-up; and maintaining follow-up logs for record keeping purposes.
• May require travel to other TUH Campuses as needed.
• This position requires the successful completion of the CHFN or CHFN-K - Heart Failure Nursing Certification, within 2 years of date of hire.

Education Bachelor's Degree in Nursing from an accredited school of nursing. Required Master's Degree in Nursing from an accredited School of Nursing. Preferred

Experience 1 year experience with a Heart Failure program Required 3 years experience in cardiology nursing Required 1 year experience in critical care nursing. Preferred
