The Social Worker identifies the social and emotional needs of clients and coordinates services between families, facility staff, and the community. They also participate in interdisciplinary treatment planning and provide psychotherapy services.
Candidates must hold a Master's Degree in Social Work and possess a current LSW or LCSW licensure. Proficiency in Microsoft Office and a current American Heart Association CPR certification are also required.
The Social Worker shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community. Assess patients and gather information from available sources, including family, agencies, friends, and others, for analysis. The Social Worker formulates and communicates treatment information to other members of the interdisciplinary treatment team; they then participate in the development of the patient's treatment plan, coordinate or provide services including individual, group, family, marital and/or group psychotherapy and discharge planning. Serves as a resource to staff and community regarding psychosocial issues. Participates in Clinic, Hospital, or departmental planning committees and teams.
