You’ll Do: Completes an initial, comprehensive and ongoing assessments of patient and family to determine home health or hospice needs. Provide respectful, compassionate care in patients’ homes. Develop patient focused care plans that establish goals based on nursing diagnoses, and incorporate rehabilitative nursing actions. Collaborate with interdisciplinary team members.
