The Social Worker provides individualized psychosocial assessments and case management for high-risk members to improve health outcomes. They collaborate with interdisciplinary teams to develop care plans and connect members with essential community and medical resources.
Candidates must hold a Master's degree and be a licensed MSW or LCSW. The role requires knowledge of case management processes, medical insurance systems, and the ability to travel for member visits.
The Regal Social Worker is responsible for assessing and providing social work services to members and their families, and is capable of independent decision-making within their Scope of Practice and duties outlined in this job description. A.Essential duties and responsibilities include the following: 1.Work with healthcare providers, case management, administrators, and other individuals involved inmember care to promote appropriate social services and medical care coordination. 2.Provide psychosocial assessment of Regal North members through home-, community- and facility-based visits, as appropriate. 3.Consult upon requests and/or accept social work referrals from the regional medical directors, case management, pharmacy team or PCP offices, regarding social service resources or information.
The pay range for this position at commencement of employment is expected to be between up to $85,000 /year for MSW and up to $90,000-$93,000 /year for LCSW annually however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Degree Requirements: Master’s Degree in Social Work, or at least
Education and/or Experience: 1. Master’s in Social Work (required); licensure is an advantage but not required. 2. Minimum 2 years of professional experience providing social work services 3. High level knowledge of Case Management processes and principles. 4. Knowledge of Medi-Cal and Medicare covered services. 5. Knowledge of HMOs and managed care highly preferred. 6. Must have experience working with a diverse member population, primarily consisting of senior members. 7. Ability to work with all levels of management. 8. Excellent communication skills, both verbal and written.
