The pharmacist is responsible for accurately verifying, preparing, and dispensing medication orders while providing clinical pharmaceutical care to patients. They also manage daily drug distribution, supervise support personnel, and ensure compliance with safety and regulatory standards.
Candidates must hold a B.S. in Pharmacy or a Pharm.D. and possess or be eligible for a Registered Pharmacist license in Connecticut. Previous hospital pharmacy experience is preferred.
The Pharmacist reports to the Director of Pharmacy, this position is responsible for providing quality pharmaceutical care for the patients of The Stamford Hospital with an emphasis on accuracy and patient safety.
37.5 hours a week. Flexibility required to work rotating day, evening, and overnights including weekends as needed.
6. Monitors patient therapy in accordance with departmental policies/procedures, protocols, and programs.
7. Carefully identifies, prevents, intervenes, and follows up on any medication related problems, including any actual or potential adverse drug events
8. Responsible for day-to-day management of drug distribution, including all controlled substances, IV compounded sterile medications, extemporaneously compounded medications, repackaged medications and investigational drugs
9. Supervises pharmacy support personnel to ensure the seamless delivery of pharmacy services throughout the institution
10. Completes required continuous training and education requirements, including state regulated, hospital and departmental specific requirement
11. Ensures safe and appropriate cost effective medication therapies for patients according to established policies, procedures and protocols.
1. B.S. Pharmacy, Pharm.D. preferred.
2. License or eligible for license as a Registered Pharmacist in Connecticut.
3. Previous experience in a hospital setting preferred.
