The physical therapist will collaborate with a care team to manage and deliver patient care plans in a home health setting. Responsibilities include traveling to patient homes and maintaining accurate, timely clinical documentation.
Candidates must hold a Bachelor's or Master's degree in Physical Therapy and a valid state license in Pennsylvania. A minimum of one year of healthcare experience and a valid driver's license are required.
