The Nurse Practitioner will provide medical care and manage terminal illnesses for patients in compliance with state regulations and hospice standards. Responsibilities include conducting physical examinations, developing individualized care plans, and collaborating with the interdisciplinary team to ensure symptom control and eligibility for hospice services.
Candidates must hold a master's degree and be a graduate of an accredited Nurse Practitioner program with at least one year of experience in hospice or palliative care. A current nursing license and a valid driver's license with insured transportation are mandatory.
