Position Summary

• As a Home Health Aide, your focus will be on helping patients with daily activities and providing personal care. Patient-Centered Care: Spend more time providing hands-on, compassionate care to patients. Reduced Documentation Burden: Use AI-supported tools to streamline clinical notes and regain personal time. Work-Life Balance: Cut down on after-hours documentation, allowing more time for family and personal priorities. Enhanced Efficiency: Experience an easier transition to technology-driven workflows that simplify compliance with care standards. Key Responsibilities: Assist patients with activities of daily living (ADLs) and monitor basic health needs. Leverage documentation tools to track care provided and report patient conditions. Build trust with patients by maintaining focus on their needs. Education and/or Experience Certified Nursing Assistant highly preferred. In states where licensure is not required, applicants must meet one of the following: They are already a licensed/certified aide; or They have completed a state-approved nurse aide training program AND have been continuously employed in that capacity for the previous 24 months Care for Who I Am is Caring for Who We Are.
