The Physical Therapist will conduct patient evaluations, develop treatment plans, and provide therapy services within a skilled nursing facility. They are responsible for maintaining accurate electronic records and ensuring compliance with Medicaid and Medicare billing standards.
Candidates must hold a valid New Jersey license and possess experience in long-term care or skilled nursing facilities. Strong organizational, interpersonal, and communication skills are required to manage diverse patient needs effectively.
Description Local Skilled Nursing Facility Now seeking a Physical Therapist
Work collaboratively with the therapy teams and Rehab Department Complete evaluations and all necessary documentation Provide recommendations supporting communications/assessments Provide therapy as outlined in treatment plans and prescriptions Offer treatments that help improve patient’s inabilities and/or challenges Independently manage workflow which includes allotting time for treatment documentation of individual and group treatments and notes in the electronic records system Familiar with PDPM Align therapy treatments and plans with understanding of Medicaid and Medicare billing
New Jersey License Background/experience in long term care (specifically a Skilled Nursing Facility) Strong organizational, interpersonal and communication skills Ability to manage patients with different types of personalities and ability levels Creative problem solving skills
