The Speech-Language Pathologist will assess and treat patients in their homes for speech, language, cognitive, and swallowing disorders. They are responsible for creating individualized therapy plans and ensuring compliance with Medicare Part B documentation regulations.
Candidates must hold an active Speech-Language Pathology license and a Certificate of Clinical Competence (CCC-SLP). A valid driver's license, reliable transportation, and the ability to pass a Level 2 background screening are also required.
We are seeking a Speech-Language Pathologist to provide in-home care for Medicare Part B patients. In this role, you will assess and treat speech, language, cognitive-communication, and swallowing disorders, creating individualized therapy plans to help patients improve communication, swallowing, and overall quality of life. Key Responsibilities: Assess patients in their homes and create individualized therapy plans. Provide skilled interventions for speech, language, cognition, and swallowing. Educate patients and caregivers on strategies to improve communication and daily functioning. Document progress and ensure compliance with Medicare Part B regulations. Collaborate with the home health/ therapy care team to coordinate patient care. Adjust treatments based on patient progress and needs.
Active Speech-Language Pathology (SLP) license in the state of practice. Certificate of Clinical Competence (CCC-SLP) required. Experience in home health or outpatient therapy preferred. Knowledge of Medicare Part B documentation and regulations. Strong assessment, treatment planning, and patient education skills. Excellent communication and interpersonal abilities. Reliable transportation and a valid driver's license. Comfortable working independently and managing a flexible, home-based schedule. Proficiency with electronic medical records (EMR) and basic computer skills. CPR and other relevant certifications as required.
